Sign Up for your Free Formed Account

  1. Go to formed.org

  2. Click Parishes

  3. Click Find Your Parish Subscription

  4. Enter Our Lady of Lourdes Church Malverne and select the first one 65 Wright Ave

  5. Click Next

  6. Create your account (Enter name and email)
